Measurement of the scapholunate distance is used as one of the indicators for carpal instability. Previ- ous studies have reported scapholunate distances greater than 2-5 mm as evidence of scapholunate dissociation. However, these studies included only adults (1). … In radiology, the Terry Thomas sign is a scapholunate ligament dissociation on an anteroposterior view of the wrist. Most commonly a result of a fall on the outstretched hand (), the scapholunate ligament ruptures resulting in separation of the lunate and scaphoid bones. This burst causes the scaphoid bone to dorsally rotate. A gap of more than 3mm is pathognomonic for scapholunate dissociation.

Scapholunate dissociation with widening of the SL joint and foreshortening of the scaphoid due to palmar tilt. 3. Arthrosis of the Radioscaphoid and Capitolunate joint due to the abnormal movements of scaphoid and lunate. 4. Dorsal tilt of lunate indicating DISI.
These findings represent scapholunate dissociation (rotatory subluxation of the scaphoid), and is the mildest form of this injury; Case B-D Case 7B showed a transcaphoid perilunate dislocation; note that the lunate and radius remain normally aligned.

2 Scapholunate misalignment on both static and stressed radiographs is defined as static scapholunate … A scaphoid radiograph series, consistent with American College of Radiology guidelines, was obtained based upon examination findings. The radiographs revealed a scapholunate gap of 4 mm and a scapholunate angle of greater than 60°, suggestive of scapholunate dissociation.
